Medical Records Request

It is the obligation of The Nathaniel Witherell to protect the confidentiality of our resident’s medical
record. Any information contained in the medical record is confidential and protected by federal and
state law.

Residents have the right to request, inspect and obtain copies of their medical records. Please see our
policy on individuals’ right to access protected health information.

Residents will be furnished a copy of their record upon receipt of a completed Authorization for Release
of Protected Health Information form. You may download a copy of the form (here)

All inquires regarding requests for access or copies of the Authorization for Release of Protected Health
Information form should be directed to the Medical Records Department by calling 203-618-4243.
