Care for end-stage renal disease (ESRD), cancer recovery, arthritis, and other deconditioning illnesses
When you are admitted to the Nathaniel Witherell for short term general medical rehab, a clinical needs assessment is conducted by our highly trained nursing staff. It includes pain management and post-op care if applicable. Typically, within 24 hours of admission a thorough physical and occupational evaluation will be completed.
The evaluation results in a prescribed individualized, coordinated treatment plan. Your team includes your surgeon, attending physician, RN patient care coordinator, certified nursing assistants (CNA), and discharge planner. Once the plan is in place you will be assigned a primary physical and occupational therapist. The objective of the plan is to provide therapy until you reach your individual treatment goals. Physical and occupational therapy is available five days a week and speech therapy four days a week. The goal is to reinstate each rehab candidate to a maximum physical and mental condition. Treatment plans include specific education related to diagnosis, pain evaluation and management, medical reconditioning, progressive strengthening, and gait and balance training. Skilled nursing care and social services are available to residents along with aid in discharge planning.
